One of the facility's employees carelessly leave little Shepard sleeping in his carseat instead of putting in a manger. She came to check on him two hours later, and when she touched his skin, he was freezing. Although she called 911 immediately, the damage was already done. Unfortunately the child died of something called "positional asphyxia".
The baby suffocated in his sleep because he was left in his car seat to sleep.

at an early age, babies do not have the strength to raise and support their own heads . If loose in a carseat, infants can become slumped over, making them unable to breathe. This act of laziness and disregard for safety Shepard from the employee of the daycare is what ultimately took his life.
The most terrifying thing about the situation is that the death Shepard does not appear in public records.
